Team Adult Advisor (TAA)/Chaperone Guidelines
Please read the TAA/Chaperone Guidelines carefully as it contains important information about the roles and responsibilities of the TAA/Chaperone.
Youth Permission Packet
All youth must receive a copy of the Youth Guidelines/Permission Forms packet, review it with their parent/guardian, receive all necessary signatures and submit it during on-site check-in at the Spring Youth Forum in order to attend.
Please ensure that your youth bring one (1) original and two (2) copies of Youth Guidelines/Permission Forms packet and bring the signed forms to the Spring Youth Forum to be submitted during team check-in. Youth will not be allowed to attend the Spring Youth Forum without bringing all necessary signed forms to the Forum and submitting them at check in.
Forms should NOT be sent directly to HCA, DBHR or UNR/COB. Bring forms to the Spring Youth Forum and submit during on-site check-in. (Each member of the team and a chaperone should also carry a copy of the form around in the event of an emergency.)
